Welcoming Meghan Hellyer to Penelakut Health

I am here every Tuesday – I am employed by island health and offer to support folks with their social determinants of health. I am taking over for Angela Potskin 🙂
The role to support with Social Determinants of Health is to apply a holistic lens to care planning, i.e. assessing the physical, emotional, social, spiritual and economic aspects of health and how that contributes to one’s overall health and wellness
Some examples of the support you will provide includes:
- Advocacy
- Systems navigation
- Case management
- Assessments/referrals
- Counselling
- Housing
- Paperwork assistance for community members
- Connection to community resources
- Funding requests
- PWD applications
- Planning (advanced care planning ect)
